Program Guidelines

  1. Patients must receive an eye examination from an eye care provider within Cherokee Nation Health Services to qualify for eyeglass services.

  2. Eyeglass services provided through the program must be redeemed at a participating CNHS Outpatient Health Center location.

  3. Eligible patients may participate in the Eyeglass Program once within a 24-month period.

  4. Patients must have a dilated eye examination to qualify for the program unless the CNHS eye care provider determines that dilation is not medically necessary.

  5. Eligibility for new eyewear is based on the patient's prescription and clinical need. Generally, patients must have a minimum prescription of +/- 0.50 diopters, a change of the same amount from an existing prescription, or significant damage or wear to existing frames or lenses that makes the eyewear nonfunctional. Final determination is at the discretion of the CNHS eye care provider.

  6. If the CNHS eye care provider determines that new eyewear will not improve the patient's vision or is not clinically indicated, the patient may not qualify for new eyewear through the program.

  7. Once eyewear has been dispensed, any costs associated with repair or replacement due to damage or breakage may be the responsibility of the patient, when applicable.
